When you get back on the air, get a copy of:
http://home.earthlink.net/~steve53/os2diags/DumpTrapScreen.zip
and use it to record and extract the trap screen data. You made a couple
typos when you recorded the data by hand. Unfortunately the missing cslim
value is often important.

What version of RSJ are you trying to install? The older versions of RSJ
were notorious for replacing a working disk driver with an older version.
If you are using Dani's drivers, make sure RSJ did not install its own
version of DANIS1506.ADD.
