It happened to me (!) with a file called prn in the root. When all
else failed, I messaged Doug Azzarito for advice about low-level
(disk editor) renaming of the file in the disk directory structure.
He suggested renaming it to the next thing in the alphabet, because
the (HPFS) directory entries are alphabetized.

I used the Graham utilities, as I recall, to search the disk surface
for the right combination of letters in the right location and then
edit what I found to read "prm". which I could then delete.

This is not for the faint of heart, but I was desperate. I wish
I could remember more about it, but it was several years ago.
I had only a very few hits, but still had to choose the most likely
candidate to edit. Got it the first try. Perhaps someone can
help you identify the right directory structure when you find it.
