You have a bit of overkill that you should clean up.  Lose the extra 
USBMON.EXE.  Lose USBQSER and USBCOM for now too.  Both USBCOM and USBQSER 
define COM devices and that might make it difficult to interpret some 
results.  At this point, I believe the USBSER has the best chance to find 
and operate the modem, but this believe is subject to change. 
The drivers list shows the same thing, if you know what to look for.

I suspect you want to install USBRes 
 ftp://ftp.netlabs.org/pub/usb/usbres.zip 
and edit the IDS file to include the AirPrime card.  It might be helpful. 
You might also consider, disabling some of the drivers you don't need at 
the moment.  It might make it easier to interpret the result. 
Once you get the driver loading cleaned up a bit. 
You might check Veit's idea that the slot is not kept powered up by 
running PCI.EXE after letting the system sit for a bit.  If the slot is 
powered down, the USB controller will not show up.
