I don't know what all you have. You said W4 with a rather old kernel.
My suggestion has always been, use the latest DANI and kernel.
DANIDASD.DMD after 1.50 does not work with some DVD-RW.

Are you sure your up2TB.flt is form Dani? Here is what I have. I am
not using it.

up2TB.flt -- huge disks support filter driver for OS/2.
Version 1.0.2

0. About.

Currently OS/2 DASD subsystem doesn't support disks with capacity more than
502 Gb. DaniS506.ADD, the great driver by Daniela Engert
(http://hobbes.nmsu.edu/cgi-bin/h-search?key=danis506), may be used for
huge IDE/ATA drives to avoid 502 Gb size limitation.
But there is no solution for large SCSI RAID devices.

up2TB.flt driver allows to use in OS/2 hard disks of any types with
capacity up to 2 Terabytes (actually up to 2032 Gb). This is not real
disk controller driver, but so-called filter driver, that works with native
hardware .ADD driver. up2TB.flt simply alters CHS disk geometry
information passed to system (actually to OS2DASD.DMD) so the whole
disk capacity may be successfully used in OS/2.

Currently only non-removable HDDs are supported.

2. How to use.

2.0. Please use the most recent versions of OS2KRNL, .ADD, OS2DASD.DMD,
JFS.IFS, etc.. Install necessary hardware drivers for your HDD controller.

2.1. Copy up2TB.flt into \OS2\BOOT\ directory of system startup drive.

2.2. Add line "BASEDEV=UP2TB.FLT" into \CONFIG.SYS.
Optional command line parameter is "/Q" - for quiet driver operation.

2.3. Reboot with large HDD plugged in.

2.4. If LVM program can't operate with your disk, use mbrutil\mbrutil.exe
command-line program to fill HDD's MBR sector with zeros. CAUTION! Use with
care! Save old MBR to file first! Run mbrutil to see usage help.

2.5. Use LVM to create volume(s) and format it under JFS file system.

2.6. Enjoy. Send bunch of money to the author.

Filter writes additional debug information into OS/2 System Trace facility.

To read it, add to your \CONFIG.SYS lines:

TRACE=ON 66
TRACEBUF=64

Copy debug\TRC0042.TFF file into your \OS2\SYSTEM\TRACE\ and run
\OS2\tracefmt.exe after reboot.
