Yes.

It's based on the ATI Radeon X800 chip, with 128 MB. It's called the
Xtasy.

Yes, I am using a board with a built-in Nforce 4 (usig GenMac). I am also
using a board with a built-in Broadcom NIC (recent native OS/2 drivers
from the Broadcom site).
My Nforce board came on an nForce 4 Mainboard by EVGA, in a small portable
barebones box from them.
